About the Role:

As a Social Worker, you will be a key member of a collaborative care team, delivering brief, solution-focused interventions that promote functional improvement. You’ll work closely with physicians and other medical professionals to develop clear, actionable treatment plans and provide expert consultation to enhance patient outcomes.

This role is ideal for someone who thrives in a dynamic, fast-paced environment, can pivot seamlessly between patient-facing and provider-facing interactions, and is committed to high-volume, high-impact care.

Responsibilities

  • Deliver integrated behavioral health services to patients of all ages
  • Conduct brief (15–30 minute) assessments and interventions
  • Collaborate with primary care providers to support holistic treatment plans
  • Provide expert consultation to medical staff on behavioral health concerns
  • Maintain adaptability and independence in a high-productivity setting

Required Qualifications

  • Licensed in Oregon as an LPC or LCSW
  • Master’s degree in Counseling, Psychology, Social Work, or PsyD

Preferred Qualifications

  • Experience in an integrated care delivery system
  • Fluency in Spanish (language differential available)

$85,000 to $120,000 per year annual salary.
