As an RN Hospice, you will play a crucial role in planning, organizing, and directing hospice care, with a focus on community health education and experience. JOB SUMMARY

The RN Hospice plans, organizes, and directs hospice care, utilizing the resources of the community to meet the needs of individuals and families within their homes and communities.

DUTIES & RESPONSIBILITIES

Patient Care

  • Complete comprehensive assessments of patients and families to determine hospice needs, including physical assessments and history of current and previous illnesses.
  • Provide professional nursing care by utilizing all elements of the nursing process.
  • Develop care plans and establish goals based on nursing diagnosis, incorporating palliative nursing actions.
  • Administer medications and treatments as prescribed by the physician.
  • Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and optional comfort care.
  • Act as Case Manager and coordinate patient care for assigned caseload.

Communication

  • Maintain accurate clinical notes regarding patient's condition and care given.
  • Communicate with physicians and community health-related persons to coordinate the care plan.
  • Teach patients and families self-care techniques and provide medication and diet instructions as ordered by the physician.
  • Provide a safe environment for the patient.
  • Work in cooperation with the interdisciplinary group to meet the emotional needs of the patient and family.
  • Attend interdisciplinary group meetings.

Additional Duties

  • Participate in on-call duties as defined by the on-call policy.
  • Ensure necessary items and services are available for patient care.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Maintain and upgrade professional knowledge and practice skills through continuing education.
  • Participate in quality assessment performance improvement teams and activities.
