Job Title: Evening Shift LPN

Location: Elmore Assisted Living – Elmore, MN

Schedule: Part-Time or Full-Time | Evenings & Every Other Weekend

Pay: $30–$40/hour (Depending on Experience)

Weekend Differential: + $1.00/hour on Saturdays & Sundays

Elmore Assisted Living, part of the Sunflower Communities family, is hiring Licensed Practical Nurses (LPNs) to bring skilled, compassionate care to our residents during the evening shift. If you're a detail-oriented nurse who enjoys meaningful one-on-one care and being part of a supportive team, we’d love to meet you.

What You’ll Do:

• Provide and oversee high-quality nursing care tailored to each resident
  
• Conduct clinical assessments and document resident health status
  
• Monitor chronic conditions and manage medication administration
  
• Communicate changes in care with residents’ families and providers
  
• Collaborate with the care team to maintain a safe, respectful environment
  
• Be a steady, familiar presence for residents during evening hours
  

What You Bring:

• Active LPN license in the State of Minnesota
  
• Experience in assisted living, memory care, skilled nursing, or home health preferred
  
• Comfort using electronic medical record (EMR) systems
  
• Strong planning, communication, and time management skills
  
• A commitment to person-centered, relationship-driven care
